Assessing Process Hazard Analysis (PHA) Quality
Plenty of energy and attention have been invested in defining the PHA as an activity: What are the best methods for carrying out this analysis? Who should take part? What are the pitfalls? How should it be structured? In fact, these are important questions to consider. About the PHA as a document or report, however, less has been discussed. To be effective, this written report needs to communicate its findings effectively to those who were not involved in the activity phase of the PHA. Since it is a nearly universal scenario that at least some individuals uninvolved in writing a PHA document are called on to interpret and be guided by its results, a group of process safety experts participating in the Safety in Action conference in Antwerp in 2016 decided to address the challenges inherent in this undertaking.
